Grade 11

  • · Original Copy of Form 138 (Grade 10 report card w/ school principal’s signature and promotion to Grade 11)
  • · Clear copy of PSA Birth Certificate Original Copy of Recommendation Form (signed by the Adviser/Principal/ Guidance Counselor) or Good Moral Certificate
  • · Recent 3 pcs 2x2 ID Picture (white background with name tag on the front)
  • · ESC Voucher (if applicable)
  • · Medical Certificate (if applicable)

Senior High School Transferees

  • · Original Copy of Form 138 (Grade 11 report card w/ school principal’s signature)
